Reporting CEUs
Self Study CEU Reporting Forms
When reporting self study CEUs, please submit your transcript from Audiology Online along with a completed self study reporting form through the CEU reporting.
*Note: certificates of attendance from Audiology Online will not be accepted, a transcript is required.
Board Approved CEU Reporting Forms
When reporting board approved CEUs, please locate the Board approved form on the list of approved courses. The approved course will list all dates and locations it was approved for.
If you already have a completed form obtained from the program provider after completing the course, please submit this completed form for CEU reporting.
If you took the course on another date or location that is not approved, you cannot submit this course for CEUs.
If the course you took was approved for the date and location you took it, but you only have a certificate of attendance from the program provider, please complete the Board approved form and submit the completed form along with your certificate of attendance.
Note: Multiple pages and items can be uploaded into one submission in Certemy when reporting your CEUs.
Apply & Submit for CEU Program Approval
All applications for CEU approval will need to be submitted through the above portal (Certemy). If you have never logged into to Certemy you will need to create an account for logging in each time to get CEUs approved.
Please use the blank application form for program approval when submitting your course for approval.
