I did all of my CEUs on Audiology Online, how do I report them to the Board?
Please submit your transcript from Audiology Online (please note certificates of attendance from Audiology Online will not be accepted) along with a completed self study reporting form through the CEU reporting workflow with in Certemy.
Do I need to report my self study CEUs separately?
Please submit your transcript from Audiology Online and the self study reporting form with in the same submission, do not submit the information separately. You can submit several uploads with in a single submission.
I attended a course and have the certificate of attendance from the course but I did not get the Board approved form, how do I submit my CEUs?
Please locate the Board approved form on the list of approved courses for 2024 on the website. The approved course will list all dates and locations it was approved for. If you took the course on another date or location that is not approved, you cannot submit this course for CEUs. If the course you took was approved for the date and location you took it, please complete the Board approved form and submit the completed form along with your certificate of attendance (in the same upload, more than one upload can be added to a CEU reporting submission).
How do I submit my CEUs?
Log into Certemy and complete the workflow. You will need to submit the Board approved CEU course form(s) (please see additional FAQ information above related to the type of courses you took) and payment of $15 per CEU reporting form (note: it is $15 per CEU reporting form not $15 per CEU reported).
