A new online renewal form is available for license renewals this year.  In addition, licensees have the option to pay the renewal fee electronically and receive immediate confirmation that the payment has been submitted to the Board office.

***Please note:  If you have difficulty scanning and attaching forms to your renewal application, you may FAX the audiometer calibration sheet and CEU reports to the Board office.  Be sure to include a COVER SHEET with your NAME and LICENSE NUMBER so the information is attached to your online renewal application properly.  Fax number is 919 834-3665.

The Board office strongly encourages the use of online renewals and payments -- everything can be submitted electronically, giving you confirmation that your submission has been received, without relying on the postal service to deliver it before the deadline.  The system has been tested and feedback has been positive.  If you experience any difficulty with the online renewal process, please contact the Board office. 

 

You must receive an electronic invoice from the Board office for the renewal fees (including CEU reports, if any) before attempting to make the online payment.  This allows a specific invoice number to be assigned to your electronic deposit.  Request an invoice now by completing this form:  www.nchalb.org/forms/InvoiceRequest.htm

 

Online payments are processed as a debit from the bank account you designate and the funds are electronically deposited into the Board’s bank account.  No credit card payment option is available.  All electronic payments are processed through the Intuit Payment Network.

PLEASE NOTE:  Even if you are not making an electronic payment, you can still use the online renewal form to submit the application.  Just check the box on the form to indicate that you will mail payment separately.  Be sure to write your license number in the memo line of the payment.
